The easiest way to create a MyChart login and password is with an activation code or activation link. There are multiple ways to obtain an activation code or link. You can still sign up without a code or link. Please listen to the following instructions. Your provider or practice may have provided you a 15 -digit code or sent you a link to sign up for MyChart. The code would have been given to you during your visit or may be printed on your visit summary. If you have a code, you can use that code for up to 60 days following your visit. You may also have received an email or text message with a link to sign up for MyChart. That link will be valid for 24 hours after your visit. If you were texted a link within the last 24 hours, find that link and click it to sign up. You will also need your date of birth and zip code to create a MyChart account. If you did not receive a link but have a 15 -digit code given to you on paper, please get it ready for the following step. If you do not have a link nor a code, you still can open a MyChart account via the MyChart website by listening to the following instructions. To sign up for MyChart with or without a code, please go to mychart .mountsinai .org forward slash MyChart forward slash sign up. Click sign up now. If you have lost or forgotten your username or password, you can retrieve or reset both by going to mychart .mountsinai .org. That's mychart .mountsinai .org. If you lost or forgot your username, click on Forgot Username. You will be prompted to enter your first and last name, date of birth, and home zip code. Please use the same name and zip code you used when you signed up for MyChart. If you enter a different name, date of birth, or zip code than on file, your username recovery email will not be sent. Click Submit. Your username will be sent to the email on file with Mount Sinai. Please check that email to retrieve your MyChart username. Please note, it may take a few minutes for email to arrive, and please ensure you check your email spam or junk folder. Please note, you have already followed the preceding steps and have not received a username recovery email for several minutes. It is likely you entered a different name.
